LCD reconsideration process - How to go about it

The Local Coverage Determination (LCD) Reconsideration Process is a mechanism by which interested parties can seek a revision to an LCD. This reconsideration process is available only for final LCDs. The entire LCD or any provision of the LCD may be reconsidered.


The following requirements must be met in order to be considered a valid request. The requestor must be qualified, the subject must be appropriate; the information submitted must be adequate, and the process for submission must be followed.

The request must identify the language that the requestor wants deleted or added to from an LCD. Requests must be inclusive of a justification supported by new evidence, which may materially have an effect on the LCDs content or basis. And when textbooks or articles are cited, copies of the published documents must be covered.

The level of evidence needed for LCD reconsideration is the same as that required for new/revised LCD development. It must be based on the strongest evidence available.

The request for LCD reconsideration must be in written form and must include the name and mailing address of the requestor in order to be valid. Telephone number and/or email address is optional and if the requestor happens to be a supplier, the supplier must be included.
